Terms and Conditions of the EY Club Lito Health Card<\/h1>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

\n\t\t\t\t\t\t\t\t\t

1.1.<\/strong>\u00a0SYNERGY DOMINATION, a single-member limited liability company (IKE), in collaboration with Lito Baby Department Stores, has jointly created the EY Club L personalised themed health cardLito Baby Department Stores, which is available free of charge to by anyone who makes a purchase from Lito Baby Department Stores or the online shop letoshop.gr. The company SYNERGY DOMINATION SINGLE-MEMBER IKE has developed and continues to develop a network of affiliated private doctors and medical centres under the name \u00abEY Club\u00bb (Health Insurance).<\/p>\n

1.2.<\/strong> Holders of the EY Club health card issued by \u2018BREFIKA POLYKATASTIMATA\u2019 can enjoy unlimited access to the medical services provided by doctors and medical centres affiliated with the EY Club. In accordance with its articles of association, the aforementioned company undertakes the development, management and promotion of health programmes on behalf of third-party natural or legal persons, as well as the provision, in return for a subscription fee, of intermediary services to facilitate the enrolment of natural or legal persons in health and rehabilitation programmes.<\/p>\n

1.3.<\/strong> Holders of the EY Club Lito Baby Department Stores health card, in order to enjoy the benefits afforded by their partnership with the store, are issued with a special card, bearing a unique identification number and valid for 60 days.<\/p>\n

1.4.<\/strong> Specifically, holders of the EY Club Lito Baby Department Stores health card are entitled, on a case-by-case basis and when visiting the network of doctors and medical centres, special preferential rates as specifically described in this leaflet.<\/p>\n

1.5.<\/strong> The company SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY, in accordance with its articles of association, does not under any circumstances provide medical services and bears no civil or criminal liability for the medical services and advice provided by its partner medical centres, which are solely responsible for the services they provide.<\/p>\n

1.6.<\/strong> The choice of one or more of the doctors and medical centres that the holder of the EY Club Lito health card would like to visit, is at their sole and absolute discretion, without any suggestion or mediation from SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY.<\/p>\n

2.1.<\/strong> Any individual aged 18 or over may become a holder of the EY Club Lito Baby Products Department Stores health card. SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY and Lito Baby Department Stores reserve the right to accept or reject, at their sole discretion, any application for membership of the EY Club Lito BABY DEPARTMENT STORES. Provided that a prospective Member\u2019s application for membership of the EY Club Lito BABY STORES is accepted by SYNERGY DOMINATION SINGLE-MEMBER IKE and Lito Baby Multi-store, the Member\u2019s registration shall be deemed to have taken place on the date the card is issued. The card will also bear the expiry date, which will be 60 days after the date of issue. The applicant will become a Member and will be entitled to the special benefits and privileges provided they receive from SYNERGY DOMINATION SINGLE-MEMBER IKE the electronic card containing their details as a Member of the EY Club Lito Baby Goods Department Stores.<\/p>\n

2.2.<\/strong> SYNERGY DOMINATION, a single-member limited liability company, is entitled to contact applicants by telephone or in person to verify their personal details.<\/p>\n

3.1.<\/strong> Membership of the EY Club Lito BABY DEPARTMENT STORES programme and any rights arising therefrom are strictly personal, non-assignable and non-transferable to any third party. 3.2. The Member\u2019s obligation regarding the use of the benefits and privileges of the EY Club Lito BABY STORES programme, as agreed between SYNERGY DOMINATION S.A. and its partner businesses, doctors, etc., is to present their personal EY Club Lito BABY STORES membership card when visiting doctors, etc.<\/p>\n

3.3.<\/strong> The Member is obliged to comply with the usual operating rules of each facility, as well as with the conditions laid down by the collaborating doctors or organisations, including adherence to appointment times for scheduled diagnostic tests, etc., except in emergency situations.<\/p>\n

4.1.<\/strong> Provided that the membership application form has been duly completed, the Member shall be granted the use of an electronic membership card, which will entitle them to the rights and privileges in force at any given time in accordance with the programme in question.<\/p>\n

4.2.<\/strong> The EY Club Lito Baby Goods Department Stores digital membership card will be sent to the email address provided.<\/p>\n

4.3.<\/strong> The Membership Card is not a credit or debit card and, as such, the cardholder-Member cannot use it to carry out commercial or financial transactions. The Member of the EY Club Lito Baby Stores is entitled to make use of the benefits and privileges, as agreed between SYNERGY DOMINATION SINGLE-MEMBER IKE and its partner businesses and individual doctors. Payment of the bill at partner businesses may be made in cash or by any credit card accepted by the business. However, in the event of future collaboration between SYNERGY DOMINATION SINGLE-MEMBER IKE and a specific credit institution or institutions, the benefits will be provided provided that the bill is paid in cash or using the relevant credit or debit cards issued by those institutions. Under no circumstances may the EY Club Lito Baby Department Stores membership card, or the offers and benefits provided, be exchanged for cash.<\/p>\n

4.4.<\/strong> The validity period of the card is determined exclusively by SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY and Lito Baby Department Stores, and is 60 days for the My Self card.<\/p>\n

4.5.<\/strong> The card is strictly personal. Members and their dependants are expressly prohibited from lending it or allowing any third party to use it. In any event, such an action, in addition to criminal liability, also gives rise to civil liability on the part of the Member and the protected Members towards the company that owns the card.<\/p>\n

4.6.<\/strong> In the event of the card being lost, the Member shall submit a new application and a new card shall be issued, valid for the remainder of the period for which the lost card was valid, unless otherwise agreed in writing.<\/p>\n

5.1.<\/strong> Collection of the EY Club card at Leto Baby Department Stores is free of charge.<\/p>\n

5.2.<\/strong> To renew the validity period of the Membership Card, the Member shall pay SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY a fee for the EY Club membership rights, as determined by SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY. Payment of this amount shall be made in the manner set out above in clause 4.2. The obligation to pay the fee for the re-acquisition of membership rights and the renewal of the Membership Card is not dependent on whether or not the benefits provided by the Membership Card are used. SYNERGY DOMINATION SINGLE-MEMBER IKE reserves the right to increase the membership fee for the EY Club programme upon renewal of the membership card.<\/p>\n

5.3.<\/strong> The fee for acquiring membership rights to the EY Club programme covers the administrative costs of the Member\u2019s registration, the costs of issuing the membership card, cash on delivery charges, the general operating costs of the EY Club programme and VAT. For this reason, under no circumstances will SYNERGY DOMINATION SINGLE-MEMBER IKE refund this fee, nor will it be set off against any claims made by the Member.<\/p>\n

6.1.<\/strong> Upon expiry of the specified duration of the Member\u2019s membership of the EY Club at \u2018Leto Baby Department Stores\u2019 and the validity of the Member card, provided that the Member wishes to extend their membership of the EY Club further, they may renew their membership by paying the membership fees again for the period of renewal, subject to the provisions of Article 4.1. The procedure for submitting a renewal application will be communicated to Members following a telephone call from SYNERGY DOMINATION SINGLE-MEMBER IKE.<\/p>\n

6.2.<\/strong> The prices listed in the statement of services are valid for one year; any price changes may apply only if the provider revises its price list for services. 7.1. The Member acknowledges, accepts and unconditionally declares that (a) SYNERGY DOMINATION SINGLE-MEMBER IKE and the Leto Baby Department Stores are at all times entitled to amend, revoke or replace part of the privileges with others, or to terminate this agreement at any time, following timely notification to the Members. Should such an amendment, replacement or cancellation take place, the Member shall not, for this reason, acquire any right to compensation, nor shall they have any other claim against SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY or the \u2018Leto Baby Department Stores\u2019 or the doctors and medicalenterprises. (b) Whilst SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY has entered into contracts with the affiliated doctors and medical enterprises, it bears no liability whatsoever for any failure to fulfil or defective fulfilment of the obligations of the affiliated medical centres, etc., or doctors within the framework of the programme in question, nor for any fault or damage that may be caused to the Member or any third party by the services and\/or products provided by them<\/p>\n

7.2.<\/strong> MELOS may be informed of any changes, additions or amendments to the network of affiliated doctors and medical centres in the following ways: (\u03b1<\/em>) By email, to the email address they have provided. (\u03b2<\/em>) Via the EY Club website. (\u03b3<\/em>) In any case, MEMBERS can obtain information by telephone via the EY Club Call Centre on 2313 253 333.<\/p>\n

8.1.<\/strong> The Member acknowledges, accepts and unreservedly declares that (a) the Member\u2019s registration and participation in the EY Club requires the creation of a personal data file, which will include the personal details and information provided by the Member upon registration or during their participation in the programme, which record will be maintained by SYNERGY DOMINATION SINGLE-MEMBER IKE and its partner company or companies for the purpose of supporting, promoting, implementation and operation of the EY Club programme. For this reason, the submission of an application for registration or renewal and the collection of the Membership Card constitute irrefutable evidence of knowledge and unconditional acceptance of the terms herein and entail thethe express and unconditional consent and agreement of each Member to SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY and its affiliated company or companiesand collaborating doctors, so that they may record their personal data in a register to be created within the meaning of Law 2472\/1997, as currently in force, Law 2774\/1999 and the Decisions and Guidelines of the Hellenic Data Protection Authority. Applicants for membership of the EY Club programme, by providing their personal data, give their unconditional consent for such data to be used to support the operation of the EY Club programme and for any futurefuture communication with them. The company complies with the EU General Data Protection Regulation 2016\/679. (b) SYNERGY DOMINATION SINGLE-MEMBER IKE is responsible for ensuring that its associates comply with the protection of such Personal Data as required under Law 2471\/97. The Member has the right, at any time, to request the deletion of their details from the EY Club database. (c) SYNERGY DOMINATION SINGLE-MEMBER IKE and Lito Baby Department Stores are entitled to use certain personal details of the Member to send theminformational\/promotional material from the EY Club and Lito Baby Department Stores, provided that the Member has given their consent to this in their membership application. Any Member may write to SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY and exercise their rights to be informed, access the personal data file, and to amend or delete the aforementioned details, in accordance with the specific provisions of the legislation on the protection of personal data.<\/p>\n

9.1.<\/strong> SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY reserves the right to unilaterally amend any term of this contract. Any such amendment shall be notified to the Member in good time and, in any event, one (1) month before the amendment comes into effect.<\/p>\n

10.1.<\/strong> SYNERGY DOMINATION SINGLE-MEMBER IKE shall not be liable in the event that, for any reason of force majeure, as described in the Civil Code or interpreted as such by other laws and the case law of the Greek courts, the EY Club is unable to provide its services.<\/p>\n

10.2.<\/strong> SYNERGY DOMINATION SINGLE-MEMBER IKE reserves the right to amend, add or remove services provided by the EY Club at its sole discretion, at any time.<\/p>\n

10.3.<\/strong> Every holder of an EY Club membership card expressly and unconditionally accepts all the terms and conditions of participation in the EY Club programme, which form an integral part of the application for membership or renewal, and the Member is obliged to comply fully with them.<\/p>\n

10.4.<\/strong> Any dispute that may arise in connection with the EY Club programme and the application or interpretation of these terms shall be settled exclusively by the courts of Thessaloniki and in accordance with Greek law.<\/p>\n

10.5.<\/strong> The online submission of an application for registration or renewal is deemed to be an accepted signed document.<\/p>\n

10.6.<\/strong> The identification code included in the electronic form (registration or renewal application) is sufficient to verify the sender\u2019s identity and the authenticity of the document.<\/p>\n

10.7.<\/strong> This contract has been drawn up following the application submitted by the MEMBER.<\/p>\n

10.8.<\/strong> It is prohibited to distribute this information or to disclose the services provided and their prices to any third party.<\/p>\n
