EY Club Health Card Terms and Conditions

1.1. SYNERGY DOMINATION, a single-member limited liability company (IKE), has developed and continues to develop a network of affiliated private doctors and medical centres under the name «EY Club» (Health Assurance).

1.2. Members of this network can, in return for an annual fee, enjoy unlimited access to the medical services provided by doctors and medical centres affiliated with the EY Club. In accordance with its articles of association, the aforementioned company undertakes the development, management and promotion of health programmes on behalf of third-party natural or legal persons, as well as the provision, for a fee, of intermediary services to facilitate the enrolment of natural or legal persons in health and rehabilitation programmes.

1.3. EY Club MEMBERS, in order to enjoy the benefits afforded by their membership, are issued with a special card bearing a unique number, valid for one year or six months.

1.4. Specifically, EY Club MEMBERS, in return for their annual subscription fee for the use of the special membership card, enjoy, on a case-by-case basis and when visiting the network of doctors and medical centres affiliated with the EY Club,centres, special preferential rates as specifically described in this leaflet.

1.5. The company SYNERGY DOMINATION SINGLE-MEMBER IKE, in accordance with its articles of association, does not under any circumstances provide medical services and bears no civil or criminal liability for the medical services and advice provided by its partner medical centres, which are solely responsible for the services they provide.

1.6. The choice of one or more of the doctors and medical centres that the MEMBER wishes to visit is at their sole and absolute discretion, without any recommendation or mediation from SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY.

2.1. Any individual aged 18 or over may be admitted as a member of the EY Club. An exception applies to dependants, namely the MINOR CHILDREN of a MEMBER who holds a Family Card. SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY reserves the right to accept or reject, at its absolute discretion, any application for membership of the EY Club. Provided that a prospective Member’s application for membership of the EY Club is accepted by SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY, the Member’s registration shall be deemed to have taken place on the date the card is issued. The card, including the Family Card and any additional cards, will be issued within a reasonable period of time following receipt of the application. This date shall hereinafter be referred to as the «start date». The card shall also bear the expiry date, which shall be one year after the start date. The applicant shall become a Member and shall be entitled to the special benefits and privileges provided they receive from SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY the card bearing their details as a Member of the EY Club.

2.2. SYNERGY DOMINATION, a single-member limited liability company, is entitled to contact applicants by telephone or in person to verify their personal details.

3.1. Membership of the EY Club programme, including status as a ‘protected member’, and any rights arising therefrom are strictly personal, non-assignable and non-transferable to any third party.

3.2. Obligations of the Member and the protected Members regarding the use of the benefits and privileges of the EY Club programme, as agreed between SYNERGY DOMINATION SINGLE-MEMBER IKE and the businesses with which it collaborates, doctors, etc., is to present their personal EY Club membership card when visiting doctors, etc.

3.3. The Member is obliged to comply with the usual operating rules of each facility, as well as with the conditions laid down by the collaborating doctors or organisations, including adherence to appointment times for scheduled diagnostic tests, etc., except in emergency situations.

4.1. Provided that the membership application form has been duly completed and the amount specified by SYNERGY DOMINATION S.A. has been paid, as set out in Article 5 below (hereinafter ‘membership fees’), the Member shall be granted the use of a membership card which will confer upon them the rights and privileges applicable at any given time in accordance with the programme in question, whilst separate personalised cards are issued to dependants.

4.2. The EY Club membership card will be sent to the address provided by courier; the member will pay the membership fee in cash and sign the relevant receipt. When the application is submitted online, the prospective Member has the option of paying by credit card.

4.3. The Membership Card is not a credit or debit card and, as such, the Member-holder cannot use it to carry out commercial or financial transactions. EY Club members are entitled to make use of the benefits and privileges, as agreed between SYNERGY DOMINATION S.A. and its partner businesses and individual doctors. Payment of the bill at partner businesses may be made in cash or by any credit card accepted by the business. However, in the event of future collaboration between SYNERGY DOMINATION SINGLE-MEMBER IKE and a specific credit institution or institutions, the benefits will be provided provided that the bill is paid in cash or using the relevant credit or debit cards issued by those institutions. Under no circumstances may the EY Club membership card or the offers and benefits provided be exchanged for cash.

4.4. The initial validity period of the card is determined exclusively by SYNERGY DOMINATION SINGLE-MEMBER IKE and is one (1) year for My Self Card and My Family Card. The Member is entitled to renew the card without restriction and each time for such period as SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY may determine. Should any of the dependants reach the age of majority (18 years), their card cannot be renewed; instead, a new MEMBER card must be issued.

4.5. Each card is and remains the property of SYNERGY DOMINATION SINGLE-MEMBER IKE, and the cardholder-Member is obliged to return it upon first request. The card is strictly personal. The Member and any dependants are expressly prohibited from lending it or allowing any third party to use it. In any event, such an action, in addition to criminal liability, also gives rise to civil liability on the part of the Member and the protected Members towards the company that owns the card.

4.6. In the event of the card being lost, the Member shall submit a new application, without having to pay the membership fees again, and a new card shall be issued valid for the remainder of the period covered by the lost card, unless otherwise agreed in writing.

5.1. Upon receipt of the Membership Card, the Member is required to pay the amount specified by SYNERGY DOMINATION SINGLE-MEMBER IKE for the provision of its services and to acquire the rights to participate in the EY Club programme for thecard’s period of validity; this amount currently stands at 100 euros for the My Self Card, 150 euros for the My Family Card for a couple, and an additional 15 euros for each minor child (including VAT). Payment of the above amount by the Member shall be made in accordance with the provisions of clause 4.2.

5.2. To renew the validity period of the Membership Card, the Member shall pay SYNERGY DOMINATION SINGLE-MEMBER IKE a further sum for the membership and renewal fees for the EY Club card, as determined by SYNERGY DOMINATION SINGLE-MEMBER IKESINGLE-MEMBER IKE. Payment of this amount shall be made in the manner set out above in clause 4.2. The obligation to pay the fee for the re-acquisition of membership rights and the renewal of the Membership Card is not dependent on whether or not the privileges provided by the Membership Card are used. SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY reserves the right to increase the membership fee for the EY Club programme upon renewal of the membership card.

5.3. The fee for gaining membership of the EY Club programme covers the administrative costs of the Member’s registration, the costs of issuing the membership card, cash on delivery charges, the general operating costs of the EY Club programme and VAT. For this reason, under no circumstances will SYNERGY DOMINATION SINGLE-MEMBER IKE refund this fee or set it off against any claims made by the Member.

6.1. Upon expiry of the Member’s specified term of membership of the EY Club and the validity of the Membership Card, if the Member wishes to extend their membership of the EY Club further, they must renew their membership no later than 20 days before the expiry date stated on the membership card, by paying the membership fees again for the period of renewal, subject to the provisions of Article 4.1. The procedure for submitting the renewal application will be communicated to Members following a telephone call from SYNERGY DOMINATION SINGLE-MEMBER IKE.

6.2. The prices listed in the benefits statement are valid for one year; any changes to prices may only apply if the provider revises its price list for services.

7.1. The Member acknowledges, accepts and declares unreservedly that (a) SYNERGY DOMINATION SINGLE-MEMBER IKE has the right at any time to amend, revoke or replace part of the privileges with others, or to terminate this agreement at any time, following timely notification to the Members. Should such an amendment, replacement or termination take place, the Member shall not, for that reason, acquire any right to compensation, nor shall they have any other claim against SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY or the doctors and medical enterprises with which it collaborates. (b) Whilst SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY has entered into contracts with the affiliated doctors and medical companies, it bears no liability whatsoever for any failure to fulfil or defective fulfilment of the obligations of the affiliated medical centres, etc., or doctors within the framework of the programme in question, nor for any fault or damage that may be caused to the Member or any third party by the services and/or products provided by them

7.2. MELOS will be able to be kept informed of any changes, additions or amendments to the network of affiliated doctors and medical centres in the following ways:
i. By email, to the email address they have provided.
ii. Via the EY Club website
iii. In any case, MEMBERS can obtain information by telephone via the EY Club call centre on 2313 253 333.

8.1. The Member acknowledges, accepts and declares unreservedly that (a) the Member’s registration and participation in the EY Club requires the creation of a personal data file, which will include the personal details and information provided by the Member upon registration and/or during their participation in the programme, which record will be maintained by SYNERGY DOMINATION SINGLE-MEMBER IKE and its partner company or companies for the purpose of supporting, promoting, implementation and operation of the EY Club programme. For this reason, the submission of an application for registration or renewal and the collection of the Membership Card shall constitute irrefutable evidence of knowledge of and unconditional acceptance of the terms hereof and shall entail theof each Member’s explicit and unconditional consent and agreement to SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY and its affiliated company or companiesand collaborating doctors, to record their personal data in a file to be created within the meaning of Law 2472/1997, as currently in force, Law 2774/1999 and the Decisions and Guidelines of the Hellenic Data Protection Authority. Applicants for membership of the EY Club programme, by providing their personal data, give their unconditional consent for such data to be used to support the operation of the EY Club programme and for any futurefuture communication with them. The company complies with the EU General Data Protection Regulation 2016/679. (b) SYNERGY DOMINATION SINGLE-MEMBER IKE is responsible for ensuring that its associates comply with the protection of such Personal Data as required under Law 2471/97. The Member has the right, at any time, to request the deletion of their details from the EY Club database. (c) SYNERGY DOMINATION SINGLE-MEMBER IKE is entitled to use certain personal details of the Member to send them information and promotional material from the EY Club, provided that the Member has given their consent to this in their application for membership. Any Member may write to SYNERGY DOMINATION SINGLE-MEMBER IKE to exercise their rights to be informed, access the personal data file, and to amend or delete the aforementioned details in accordance with the specific provisions of the legislation on the protection of personal data.

9.1. SYNERGY DOMINATION SINGLE-MEMBER LIMITED LIABILITY COMPANY reserves the right to unilaterally amend any term of this contract. Any such amendment shall be notified to the Member in good time and, in any event, one (1) month before the amendment comes into effect.

10.1. SYNERGY DOMINATION SINGLE-MEMBER IKE shall not be liable in the event that, for any reason of force majeure, as described in the Civil Code or interpreted as such by other laws and the case law of the Greek courts, the EY Club is unable to provide its services.

10.2. SYNERGY DOMINATION SINGLE-MEMBER IKE reserves the right to amend, add or remove services provided by the EY Club at its sole discretion, at any time.

10.3. Every holder of an EY Club membership card expressly and unconditionally accepts all the terms and conditions of participation in the EY Club programme, which form an integral part of the application for membership or renewal, and the Member is obliged to comply fully with them.

10.4. Any dispute that may arise in connection with the EY Club programme and the application or interpretation of these terms shall be settled exclusively by the courts of Thessaloniki and in accordance with Greek law.

10.5. The online submission of an application for registration or renewal is deemed to be an accepted signed document.

10.6. The identification code included in the electronic form (registration or renewal application) is sufficient to verify the sender’s identity and the authenticity of the document.

10.7. This contract has been drawn up following the application submitted by the MEMBER.

10.8. The distribution of this material and the disclosure of the services provided and prices to any third party are prohibited. This document is intended solely for members of the EY Club.
